Please complete the following form to verify your eligibility for Hualalai 'Ohana Foundation programs and awards. Applicants must be employed within Hualalai Resort for at least 6 months and must work for at least 20 hours per week in order to be eligible for program support. Eligible applicants include independent contractors, service providers, vendors and employees of the Four Seasons Hualalai.
