The Hualālai ‘Ohana Foundation offers Medical Financial Assistance to cover expenses incurred by the employee or immediate family members beyond those covered by medical insurance. We offer 3 different types of medical financial assistance - Surgical Assistance Award ($3,000 cap), Dental Surgery Award ($1,000 cap) and Serious Medical/Life Threatening Illness (ranges).
Awards are provided to assist full-time and part-time (more than 20 hours per week) employees and their immediate family (spouse, dependents) after six months of employment. Dependent age cap is 26 years old. Applicants are required to complete an EE verification form and submit with all applications.
These programs are offered year-round. Incomplete application packets will delay processing and review of your award. Please be sure to submit all required documents in order to expedite the application process.
Once all documentation has been received, the Programs Office will review your application and present to the Medical Committee for approval. Once approved, you will receive an award letter, notifying you of the committee's decision. The Medical Committee meets quarterly. Medical billing and reimbursements are processed in the order they are received.
This award provides support to address a non-life-threatening but disabling condition. You may be eligible to receive up to $3,000 in assistance to help pay for medical treatment and prescription co-pays as well as related travel expenses. Cosmetic surgeries or non-disabling conditions are not eligible.
We require that applicants submit complete applications including a medical diagnosis and prognosis from your doctor describing the medical situation in detail. We will need to know if the procedure is medically necessary and what the specific treatment plan is. We will also need to know what portion is insurance eligible and what is not.
Applications accepted year-round.
We are proud to be able to provide support for medically-necessary, urgent dental procedures. Reimbursement with original receipts. Non-urgent procedures / cosmetic procedures are not eligible.
We require that you submit complete applications, including a detailed treatment plan from your dental provider, description of estimated costs and what is covered by insurance. Applicants must show proof of insurance. In the case of orthodontic services, implants and services not offered by insurance plan, provide us with a statement from dental provider verifying that the procedure is not a cosmetic one and/or a statement necessitating the benefit to quality of life. Effective January 2024, the cap for dental award applications is $1,000 per year. The Foundation is proud to match the employer insurance benefit amount.
Medical awards cover expenses incurred by the employee or immediate family members beyond those covered by medical insurance in cases of serious or life-threatening illness. Two surgeries per calendar year.
Typical expenses that the Foundation reimburses are airfare, hotel, appointment co-pays, prescription co-pays, per diem, pre and post-surgery office visits. Award amounts depend on the diagnosis, prognosis and case review. We encourage applicants to keep all receipts and hospital discharge paperwork so that we can assist you.
